misplaced Passport

Hi guys. kinsa ninyo naka experience nga na misplace gyud completely ang Passport then wala gyud kay photocopy ato and kinahanglan nimo e renew kay expired nah. unsa process ani? me suway kog agency kinahanglan paman police report nga dugay plus aff. of loss then busy ayo akoa sked. naa mo nahibaw-an na agency na maka process ug misplaced passport nga dli na mag police report? .. need help

"Lost Passport" is not a simple case man.

Mas maayo jud siguro nga mo-follow nalang jud ka sa procedure sa DFA ana Boss.
That will be your best option.

Between "busy kaayo" and "lost passport", ikaw ra jud makapili og asa imo gusto.

mao na jud, affidavit of loss..kung dili ka muagi ani mahulog nga illegal imo passport..sakripisyo jud ta bai

hala.. wala paman nuon. pero try to consult sa DFA ani.. mahog naman sad nag LOST PASSPORTS gud.. anyway, DFA is only d one who can entertain ur complaints.

REQUIREMENTS FOR A REPLACEMENT OF LOST E-PASSPORT
* PERSONAL APPEARANCE IS REQUIRED *
• Duly accomplished application form with Telephone Passport Express logo.
• Original Birth Certificate on National Statistics Office (NSO) security paper.
• Affidavit of lost.
• Police Report, if valid.
• Any two (2) of the following valid IDs:
Social Security ID/Government System Insurance ID
Computerized Voter’s ID
Professional Regulatory Commission ID
LTO Driver’s License with OR
School ID with original registration card/transcript of record
Any old documents with date and place of birth

NOTE: There will be a 15-day clearing period prior to the processing of application for replacement of a lost valid passport.
