The customer will receive a unique promo code to be validated by an AXA Financial Partner after a 10-15-minute assessment of health coverage

Only confirmed and validated promo entries will be included in the raffle draw

E. TO DETERMINE WINNER
A random winner will be drawn via electronic raffle system at the AXA Marketing Division Office located at 35F GT Tower International, 6813 Ayala Ave. cor dela Costa, Makati City at 5 PM on December 10, 2014
The winner will be advised and be given instructions on claiming procedures through a call or e-mail
Employees and financial partners of AXA Philippines and all its affiliates, advertising agencies, contractors and their relatives up to the second degree of consanguinity or affinity are disqualified from joining the promotion
F. REDEMPTION PROCEDURES
Go to AXA Philippines Marketing Division Office and present the email confirmation of validated promo code together with two (2) valid ID’s with picture and signature.
Acceptable valid ID’s: Driver’s License, Birth Certificate, SSS/BIR/Philhealth/Pag-ibig, Senior Citizen, Valid Passport
Authorized AXA Philippines Marketing Personnel will issue a certificate endorsing the winner to claim the Peugeot 3008 at the nearest dealer
The winner is given 60 days upon receipt of the e-mail and/or call, to claim and present all the required documents. After 60 days, AXA Philippines has the right to transfer/sell/dispose the Peugeot 3008
Associated costs, taxes, insurance, etc. will be for the winner's account
