First you'll have to draft your statement of the problem. This will allow you to know your objectives and foresee the direction of your research. Depending on your problem, know what variables you intend to justify or what you intend to use. As soon as you have your variables, you can make them your categories in drafting your questions. The type of questions can be open-ended or close-ended questions. This will matter if you want objective or subjective answers. Questions shouldn't also be biased. This could lead to a false-positive or false-negative answer.

Once you have finished configuring your questions, you can pre-test the questions to a small sample to test its validity.

Depending on your study, there are ready-made, standardized statistical tools that don't need to be validated. On the other hand, your researcher-made questionnaire will need to be pre-tested for validity. If you can't find a standardized tool on the internet, visit your local library and borrow a thesis or dissertation.

The type of questionnaire you are going to make should also conform to the type of research you are doing or the type of presentation you will be making. This could be quantitative or qualitative in nature. It could also be categorized as descriptive or inferential.

Personally, I think the professor should have discussed the topic first or you could have taken up quantitative or qualitative research procedures first. On another note, there are a lot of half-baked master's degree holder or doctor's degree holder. The reason you're taking up the program (supposedly) is to improve yourself and to help your stakeholders through research. A lot of people are taking up post-grad programs nowadays to further their financial gains or for promotions. Don't be that guy.

Go to the library and research.
