I'm not sure if this still applies...

    - you make an affidavit of Loss/destroyed plate number
    - present this to the LTO with you OR/CR
    - they do some inspections of your car
    - they issue you an official receipt

The way back then fee I think was:
    - Php150.00 lost plate
    - Php10.00 LRF(Legal Research Fund)
    - Php100 Processing fee
    --------------------------------------
    - Php260 all in all.

They will then give you a paper/authorization for you to use a Temporary Plate number that you'll make and use it as replacement for the lost/destroyed Plate No.

Note: This info of mine is veeeeerrrryyy old...circa 2002 pa when I was a temporary Clerk in LTO: Pampanga so I dunno if the details still apply

The rates now may be higher now since the LTO is now computerized.

Regarding making a Plate number...dunno if the ones being made at The Maze: Ayala Center Cebu can be used as temporary plates. Better ask them or the LTO itself.

One customized License Plate costs Php500.00 or less. Ask them if they can make it on a "lata" type
