    MODEL RELEASE FORM


    I, _____________________________________________ (Model), give to Eisen Job C. Alquiza (Photographer),
    his legal representatives, successors, and all persons or corporations acting with his permission, unrestricted permission
    to copyright, use, and/or publish photographic portraits, pictures, or images of me taken on ____________________,
    and the negatives, transparencies, prints, or digital information pertaining to them, in still, single, multiple, moving or
    video format, or in which I may be included in whole or in part, or composite, or distorted in form, or reproductions thereof,
    in color or otherwise, made through any media in a studio or elsewhere for art, or any other lawful purpose.

    I hereby waive any right that I may have to inspect and approve the finished product or copy that may be used in connection
    with an image that the Photographer has taken of me, or the use to which it may be applied.

    I further release the Photographer, or others for whom he is acting, from any claims for remuneration associated with any form of
    damage, foreseen or unforeseen, associated with the proper commercial or artistic use of these images.

    I understand that any photographic portraits, pictures, or images of me created by the photographer which are not recognizable may
    be used with impunity, in any circumstance or context, with no remuneration, considerations, requests, or permissions required.

    I acknowledge that the photography session was conducted in a completely proper and highly professional manner, and this release was
    willingly signed at its termination. I certify that I am not a minor, and am free and able to give such consent.

    actually, the long form is better, because it covers everything that should be released to the photographer. the short form's reference to "all rights to reproduce" is actually limited as it covers only the "right to reproduce". a lawyer imaginative enough can punch a hole through this in case of dispute...

    of course, as abort said, models would find the long form intimidating since they'd need a lawyer to explain to them what it really means.....

    got this format from somewhere sa internet and have used it since

    Adult Model Photo Release

    For good and valuable consideration, the receipt and legal sufficiency of which is hereby acknowledged, I________________(“Model”) hereby grant to_____________and PHILDIGITA (collectively, the "Photographer"), the Photographer’s assigns, and those persons acting with the Photographer’s authority and permission, the right to take and create photographs (in all formats) and other graphical depictions incorporating my likeness, in any and all media, whether now known or hereafter created (the “Photographs”).

    I hereby agree that all rights in and to the Photographs, including the copyright, are and shall remain the sole property of the Photographer, free and clear from any claims by me or anyone acting on my behalf.

    The Photographer’s rights include, but are not limited to, the rights, in perpetuity, to:

    1. Use, re-use, publish, and re-publish the Photographs for PHILDIGITA;
    2. Alter, modify or otherwise change the Photographs, in any manner the Photographer desires;
    3. Combine the Photographs with textual matter and/or with other pictures and/or media; and,
    4. Use the Photographs for folio, promotional and advertising purposes of PHILDIGITA, or any other similar purpose whatsoever.

    I hereby release, discharge, and agree to hold harmless the Photographer, the Photographer’s heirs, legal representatives and assigns, and all persons acting under the Photographer’s authority or those for whom he/she is acting, from any liability by virtue of any use of the photographs or any changes or alterations made thereto.

    I hereby warrant that I am of full legal age and have the right to contract in my own name. I have read the above authorization, release, and agreement, prior to its execution, and I am fully familiar with the contents thereof.     street photography has a lot of grey areas in terms of legalities... but basically, yes you're right.

    although, if you single out a person as a subject then it could be interpreted differently.

    I've heard of a US based photographer who got sued by a homeless man for using a picture
    of him in the photographer's exhibit. although everyone seems to be sueing everybody there
    just to make a fast buck.

    Well, if you think you need to publish your work then IMHO its not hard to just approach the subject and say "Hope you dont mind, I was taking pictures of you.. blah blah..".. Really depends on what you intent to do next with your photos.
