Please Help: Lost Passport

Guyz i need help. Ani ni xa. Akong passport kay tua sa agency nga akong giapplyan before. I was supposed to work na unta sa Malaysia. I have the papers na, working Visa and all pero gi hold sa DFA akong Visa for the reason nga gamay ra daw ang sweldo for the position nga gi offer sa ako (English tutor man gud tako didto). Then na busy nako coz I started working in a call center naman gud so wala na nako na follow up, plus the fact nga akong phone kay nawala, which was my means of communication unta sa agency. 2 years ago, nibalik ko sa agency to find out if naa pa akong passport and then ni assure man ang manager nga naa pa sa ilang file (though wala ko kakita sa passport). Confident lage nga naa ra akong passport sa ilang office, just in case naa hiring matawagan rako. Last June man cguro to, I went to that agency again to get my passport na jud kay ako unta i renew. To my surprise nag stop naman diay kuno og operate ang mao nga agency and they rented another building na daw. Good thing kay nakahibalo sad ang guard sa place nga ilang gibalhinan. So mao to, i went there and told the in-charge(don't know unsa iyang position) regarding my concern. To my dismay, pag check sa file wla naman didto akong passport. WTH?! meaning nawala nila akong passport? Then I told him basig inyong gipagamit, sir? then he said dili daw na possible and dili daw nila pwede buhaton ingon ana. Instead he told me to go to DFA and file nalang daw affidavit of loss.

Now my questions are:

1. pwede pa ba kaha ko maka file og affidavit of loss nga last year pa raba diay to nawala?
2. if so, how much will it cost me man sad to get all this done?
3. I have a photocopy of my passport with it's number and info. Can I use this upon filing for affidavit of loss?
4. Kinsa man akong duolon regarding ani nga case? Who in particular sa DFA?

- The DFA said it has prepared an Affidavit of Loss or Damage which will be available free of charge to the applicant at the Office of Consular Affairs’ (OCA) Information Counter.

- yes that would be helpful

Para maka-tipid ka.. paghimo ug affidavit of loss daan... dad-a sa may cityhall kanang kilid nya pa notaryo lng.. 30pesos ra.

mas dali ang process basta expired na imong passport...

pagdala rag nbi.. nso birth... and id's.. dali ra ang process ana basta expired na imong passport..
basta online lng ila system.. ila ra to i-verify then process naka same as kanang mo renew...

tip.. mas ok moadto mga 9am sa dfa.. dali ra man ang linya.. di kaayo dugay ka magpaabot compared kung mosayo kag 5am anya 8 pa moabli.

btw, this is granting imo passport was issued here region7 dfa.
