I lost my dog's Registration and Pedigree Certificates. How do I obtain another one?
You can download the "AFFIDAVIT FORM" from our website. Then you will have to fill-up and sign the form and send it to the office along with your payment for the Duplicate Copy amounting to Php110.00

I just lost my dog's PCCI certificate that I obtained from the previous owner and I did not transfer the ownership into my name. How do I obtain another certificate?
If you lost your dog's certificate and the dog was not yet transferred into your name, the dog's recorded owner will need to fill-up the PCCI Affidavit Form and the Supplemental Transfer Statement Form. Please include the following information:

o Dog's name, registration number and breed
o Recorded owners name, address and signature
o Php110.00 processing fee (you can either pay by check or money order made payable to the PCCI.
o Php250.00 transfer of ownership
o Php33.00 supplemental transfer

If they do not have the dog's name and registration number please provide us with the following in its place:

o Names and registration numbers of the sire and dam, and breed
o Date of birth of the litter
o Recorded owner's name and address

When the required information is received a new certificate will be mailed to you as the new owner.

Please remember a duplicate certificate can only be requested by the current owner on PCCI record.

I mean, when you will not pass na in any mediator like caminadae petshop who can also process

i heard na it takes daw about a month or two. wala pa ko katry though, I am still on the verge of getting my husky's ownership transferred to me.

sa Caminade ka mag pa process sir? kay magpa transfer pa sad ko og ownership at the same time mag kuha og affidavit form..

Dili lang tingali eheheh. The previous owner of the GR puppy I bought yesterday are going to Manila on March to process their papers. Magpapasama na lang ako sa kanila. I heard kasi na 1k plus daw ang bayad kung sa Caminade. Kung magpapasama ako, I would only pay about 800. Here is the breakdown: Php40 - identification card, Php500 - membership, Php260 - transfer of ownership. Dapat may 2 1 x 1 pic for the ID.
