if transport via plane or boat, look for the BAI or Bureau of Animal and Industry, in common parlance they usually call it on the port as "quarantine". usually P50 ra na, pero hatagi lang ug P100
as you mention on the thread title "ship" i assume its a boat you will be travelling with. BAI is open office hours, so if your departure time is off the office hours, then better go there sa pier during office hours; look for BAI then have your quarantine document signed. 7 days validity ng quarantine certificate, so its okay if you can have the certificate days before your departure.
BAI usually checks the health card/record for vaccinations and anti rabies shots. they dont mind the PCCI or the pedigree registration of the dog(even if it has any).
it is required though that you will have it caged or on a portable kennel. shipping lines dont allow transporting dogs or cats without cage or portable kennel.
