- Get Affidavit from a Notary Public
- Go to RTC (Regional Trial Court~~Judge) sa Capitol for Notarial Act
2. NSO
- Secure Copy of Birth Certificate from LCR (Local Civil Registry)
- copy of NSO birth certificate
3. PRC
a. Certification
b. Board Rating
c. License
- Just secure certificates
-----> Then, go to WWWE and pwede ra ipada all at once basta under the same name. P500 per page for the first document and the succeeding documents will cost only P150/document.
