Default Need Your Inputs In A Quite Confusing Situation

The mother of my partner died recently. The son was about to receive the death benefit of the mother's (since his mother had pension) from GSIS when found out that it is deducted due to the last pension was withdrawn. The son is very confused due to there is no person withdrawing using the card nor he gave the card to anybody (he is very careful of that card and never let anybody have it). The last time the son checked the ATM card was after his mother's death but there was zero balance, thus the last pension was not released. He went to GSIS though for the purpose to report the death of his mother in relation to the pension that his mother's getting every month. GSIS adviced him to go to UNION Bank (the bank of his mother for the monthly pension) so thus he did. But UNION Bank informed him that there was a withdrawal of pension happened as per their computer records said. Though UNION Bank said that he can request for the investigation of the situation, since the son knows clearly had not done any withdrawal due to the last time he checked the ATM card, it was zero balance.... The son is quite confused about this situation. Is glitch possible to happen with this?

What are your advises guys? Your inputs might help him out... So as I also is quite confused about situation because I know he never withdrew recently from that ATM card due to the last time he checked it (for the purpose that he was supposed to be going to use the money for the expenses in his mother's wake and funeral), it was of zero balance.

get as much details as you can about the said transaction bro,
have the bank investigate it as there might be have been a fraudulent activity that took place
or perhaps just some system error that can be fixed straight ahead..
or kung unsa man gani, consult mo ug lawyer.

But UNION Bank informed him that there was a withdrawal of pension happened as per their computer records said.

This is what you have to trace,
    who made the withdrawal?
then records are there as what they said,
ask for a printed copy, and of course with their signature
on it to certify it as true copy of the bank statements.

    example lang ni ha...

SSS and GSIS pensions karon kay kinahanglan nag appearance ang senior citizens/disabled to make a personal appearance EVERY year...
this is done to prevent or at least to lessen fraudulent activity within the account...
i know this how? akong papa ug lola pensioners...akong uncle, disability pensioner...and ako ang mo-drive nila padung aning mga lugara...ako pod mo-kuyog, mo-process...
this way, kabalo gyud mi sa activity sa among accounts...PLUS pa gyud, i-enroll nako online ilang accounts para makabalo ko sa movement...

ang imo TS, since murag naa na'y kahibulngan nga aktibidades, pagdala nalang ug lawyer/friend para at least naa siya hibaw-ana or unsa...

mao daw ni buhat sauna...inside jobs anang mga pensioners nga wa kuhaa...sila mo-trabaho...

Thanks a lot in your inputs guys; All noted. As of now, he is securing a true copy of his Birth Certificate to be submitted to the Union Bank so that the documents with the full details of the transaction shall be processed (for tracing up / Bank wont process the transaction document without the true copy Birth Certificate of the son). Plus the personnel in Union Bank said there shall be a surveillance camera shot upon investigation...

The amount of money which was, I may say, somewhat stolen or gone is not that very big (11-12k), so as with the deduction with GSIS, but it is still money in a certain amount. Lawyer is a very good idea indeed but it might imply expenditure more than the amount concerned in the situation.

Naa diay binuang (inside job) like kawat2x ani before? (in the Bank or in GSIS?)
