Request for Payment Hospitalization

• Letter of request addressed to the Hon. Chairman and/or General Manager
• Original/or certified true photocopy of medical abstract duly signed by the attending physician. (Note: Include physician's license number)
• Statement of Account/Hospital Bill certified by the billing Officer/Credit Supervisor.

- so does this mean muduol ko sa PCSO ug maadmit na ang baby? Kay magkinahanglan man ug Statement of Account. Di man sad na sila basta2x mohatag siguro if walay proofs sa amount nga mabyaran diba? Ug muadto ko karon ang abstract ra man ang akong madala.

kahinumdom ko sis katong na CS ko kay akong mama niduol tog social worker for assistance for the hospital bills kay kulang man to among money aron ko ma discharge...then niadto akong mama sa social service office sa hospital ana xa nga magpa tabang mi sa foundation ni Nerrissa Soon. gi evaluate pa akong mama, then nag issue ang social service officer og letter of endorsement para sa foundation ni nerrissa...nakalimot ko sa details gud pero sure ko nga naka avail ko og tabang gikan sa foundation ni nerrissa..

sis, kahibaw man ko anang in-ana pero inig evaluate mo-ask man na if naay work diba? then naa man ko..basin mobalibad sila kay naay work. Basin i-prioritize nila ang walang-wala jud.
