as of march 14, 2011 inquiry ..

Lapu-Lapu City Hospital
the requirements:
- Resume with 2x2 picture
[requirements below are in photocopies only]
- TOR
- Board Rating
- PRC License Card
- Diploma
- Recommendation from School
- Brgy. Clearance
- Police Clearance
- NBI Clearance
- SSS E1 with number [if available]
- TIN number [if available]
- certificates with seminars attended [optional]
- Application letter
you can submit your requirements to either of the ff. staff:
Mrs. Zaida M. Bascon [ward supervisor]
Mrs. Grace C. Estremos [chief nurse]
but beforehand, if taga LLC mo, you have to go to your barangay hall and get a recommendation from the brgy. captain, addressing it to the Mayor of LLC.
Forward your requirements to the Mayor's Office, and the secretary will check your credentials and present it to the mayor.
Once you're done, submit your requirements to the above mentioned staff.. they will tell you when to come back for an interview.
If dili mo taga LLC, try asking the above mentioned staff, if they'll accept then submit it to them.
Shifting is now 12 hours, instead of 8 hours. 6am-6pm, then 6pm-6am the next day, then 2 days off..
