What additional requirements are needed for the self employed? How much is the annual fee for the succeeding year?

for self-employed:
- a copy of Audited financial statements with bank's or BIR's stamp
AND
- copy of your SEC or DTI business registration

but if you have existing credit card with other companies (1 year or more), no need of the requirements above-mentioned.
