Just sharing my workflow ...
1) Cleanup the image using spot healing brush. Remove prickles, and other facial imperfections ... but do not overdo it!
2) Create a copy of the background in a new layer ... call it "surface blur".
3) Apply surface blur on the image Filters->Blur->Surface Blur. Adjust settings to desired taste.
4) Create a mask on the "surface blur" layer. This can be done by dragging the layer (Alt+click) to the mask icon at the bottom of the layers pane.
5) Make sure that the mask is selected on the layers pane. Using a soft brush, paint on the areas in the picture where you want it to be soft - the skin in particular, you may want the eyes, lips, eyebrows to be sharp while the rest of the skin is soft so avoid painting on them.
6) Adjust the opacity of the "surface blur" layer. This is to preserve the details that is on the background. You don't want the image to look very artificial, I normally would use 60-80% opacity. But that depends entirely on your taste.
7) Merge visible layers. This can be done by right-clicking on the layers pane and selecting "Merge Visible Layers" on the pop-up menu.
Make the eyes sparkle. The quickest way is to select the background layer (Now there's only one after the merge), and apply "Unsharp Mask", do this twice or a number of times to desired.
9) On the history pane, select the "merge visible" step (this will bring you to the last step before the "unsharp mask" was applied), and right click on the box where the last step (unsharp mask) is - this will put a brush on the box next to it.
10) Press "Y" once. This will select the history brush, on the brush toolbar, select a radius that is large enough to cover the eyes "pupils" - and then Paint on the eyes (this will make the pupils of the eyes sharp - making it sparkle).
11) Save! And that's it ....
