These are the requirements:
1.) PROOF OF INCOME - at least one but if you could submit more, much better.
(i.e. latest one month payslip, certificate of employment, W2, credit card billing statement**)
**existing for more than one (1) year

2.) Tax Identification Number (TIN)

3.) Photocopy of Two (2) Valid IDs

4.) At least one (1) Landline* AND one (1) Mobile No.
*can be a residence OR office number

5.) if employed, preferably a REGULAR employee of the company.

6.) Properly Filled-Out Application Form

>>with gross monthly salary of at least Php12,000.00*
* if it is below the said salary requirement, we can still have your application processed provided that you still secure the necessary requirements. Who knows? you just might get approved.

>> if you have a Metrobank account, don't forget to write down your account number in the credit card application form.

>> processing time: 2-3 weeks
