It would be a silent, lingering death for thousands if radioactive material were released into the atmosphere. See Chernobyl as the prime example.
If there was a reactor meltdown the extreme temperature could in theory see the core burning a massive hole through the ground with nothing to stop it. The movie "The China Syndrome" took it's title from the belief that if a meltdown occurred at a US plant the core could sink through the Earth and come out in China. Scientists now believe that theory unlikely, but the potential for environmental damage would certainly be enormous.
