The store itself is located at the North Reclamation Area near Cebu Doc. Their office (temporary) is located along Hyundai NRA going to the Wireless Bridge heading for Mabolo.

Its very near Cebu Doctors University. Fill-up the application form situated near the entrance. No need to present your billing address, just 2 valid ID's and P750, then they process it and in a few minutes they will give you your plastic membership card already w/ your picture on it.
Their items are Imported, yet affordable.
