You will have to forward your request directly to the NSO. You may submit your intention to apply for a copy of your corrected/annotated birth certificate at this e-mail address: L.Hufana@census.gov.ph. Please be advised that you will be required to submit documents pertinent to your request.

Payment and delivery handling are to be arranged directly with the NSO. You will be receiving instructions through e-mail.

Processing will take 15 working days, upon receipt of your documents. Delivery of document results will be through NSO’s local courier for destinations within the Philippines.

    i hope this helps...
i did a letter to L.hufana last month March 12,2008.... just today I receive sa iyang reply to my letter regarding my problem...

she said that there were two names registered in their database... but in civil registration law states that in case of double registration, the first registered may it be erroneous is the one that will be issued...

if i insist to use the name im using right now, i should file a petition in court for the cancellation of my first registration of birth...

i would like to seek help from u bro.... especially in the last part sa iyang giingon nga if i insist to use my current name which i was able to use in my school records, board exam, diploma, SSS, NBI baptismal, birth certificate, tanan-tanan... i will have to file a petition in court....
how to do it?... nya by how much kaha ni bro? and how long will it takes? any idea? nya better if u hav atty's in mind nga makabarato lang unta ko.... need help kaayo guys.... salamat daan..

it is better to find yourself a good lawyer for your problem. im not a lawyer and i am not at ease recommending u one. u see, diff. people have diff. expectations...it is best that u find an atty that u know, a friend, a friend's friend, or perhaps a relative, a relative's friend or relative...better if he/she will work for you "pro-bono"...besides, it is an easy job for an able atty. it's more on documentation work lang...

unsa man jud ang sakto? is it the NSO? much better kon SAKTO ang NSO kay ang NSO lang ang ipadayon... pwede raman ka makacorrect sa imong school records and all... nya sa SSS ang imo ihatag kay katong authenticated sa NSO lang, kay gamhanan kaayo ang NSO record compared sa LCR ug uban pa.... mao ni nahitabo nako, moagi pa kog petition sa court... para lang akong name nga gigamit karon maoy akong ma-use...

hi! nikuha mi ug nso marriage certification tru census but we only got "negative certificate of marriage". naay instructions ug asa mi adto. first is, civil registrar for verification, etc. my question is, i am currently working office hours. akong gusto unta, if mu leave man gani ko ug one day to go to civil registrar, gusto ko kausa ra ang pag adto. unsa pa kaha kinahanglanon ana aside sa gipadala sa census na mao pud ang giingon sa instructions? basin pud ug mag balik2x ko ana ba..
