NSO Authenticated Birth Certificate with receipt
NBI Clearance or Baptismal Certificate
3 pcs Passport size picture (To be taken at DFA
during appearance)
Valid ID with picture

*NSO authenticated Marriage Certificate with
receipt for female married applicant
*If born outside Region 7, additional requirement
for proof of residence
1 of the following:
Voters ID
If Student, School ID
If Working, Employment Certificate
*Lost Passport Requirement: Affidavit of Loss
If still valid, additional Police Report
*Requirement for minors: Additional photocopy of
parent’s passport

FEES : Regular - P 1,500.00 (25-30 Working days
upon filing)
Rush - P 1,800.00 (15-20 Working days
upon filing)
