hey i feel the same way for those who has problems with birth certificate. My name was changed already but the fact the record in the NSO still remains the same, i couldnt help but be frustrated. i sent email to the help desk officer of NSO but then, there are millions and millions of processing! i don't like how our goverment employees will work. It seems to me that they will act only if they will be given money.

Error in your name is covered under RA 9048. Instead of going to court (which was the procedure before RA 9048 was approved), you can now file the petition for change of name at the city/municipal civil registry office (C/MCRO) of ur birthplace. you hav to submit the following:
a. certified machine copy of the birth record containing the entry to be corrected;
b. not less than two(2) private or public documents upon which the correction shall be based;
c. Notice/Certificate of Posting (not yet available at the time of application);
d. Payment of three thousand Pesos (P3,000.00) as filing fee (present official receipt of payment);
e. Other documents (clearance from authorities) which maybe required by the C/MCRO. You have to visit the concerned C/MCRO for the filling up of forms.
f. Proof of publication
If your present residence is too far from her birthplace, you may file the petition at the C/MCRO nearest your residence in which case an additional Five (P500.00) Hundred Pesos service fee shall be charged by the petition-receiving C/MCRO. The steps are similar except that the filing fee of P1,000.00 shall be in the form of Postal Money Order (PMO) payable to the C/MCRO which keeps a record of your birth, and that posting (10 consecutive days) shall be done twice, one at the petition-receiving C/MCRO, and another at the C/MCRO (Record-keeping C/MCRO) which keeps her record. You can also visit the C/MCRO of your present residence for other details.
