3. Duly Accomplished Application for Driver’s License

D. LICENSE RENEWAL (PRO or NON-PRO) – Ph 417.63
1. Current Pro/Non-Pro Driver’s License
2. Duly Accomplished Application for Driver’s License
3. Medical Certificate with Official Receipt
4. Negative Drug Test Result
