Requirements for NLE application:

- NSO [original and copy]
- LCR [original and copy]
- TOR [original and copy]
- Cedula [current]
- OR & DR cases [original and all signed by the staff nurse, CI, and the screening committee]
- PRC forms, all signed by the assigned CI, staff nurses, chief nurses, and the dean
- 900 pesos for the General Fund
- 2 metered stamp, mailing envelope [c/o PRC]
- 90 pesos for the picture [1x1 and 2x2, c/o PRC]

basta pag dala lang ka ug 1.5k for the payment, you'll never know what will happen there. and also be early in filing kai nag anam2x na ug file ang June takers..

@ mets: pag andam pud d i ug passport, basig foreign sounding imo family name or imo middle initial, pangitaan dayun ka ug passport sa prc.

@mets: as tarantado has stated, also bring your voter's i.d, just in case you have a foreign-sounding surname...

your PRC forms must be clean kai lain baya nag utok ang taga PRC, dapat walai kuris-kuris... adah bahala sila oi, dghan gani kog mga erasures kai dghan changes...

but right now, make it clean as possible.

metz, dili katong RAW form ang i-pass.(katong naa sa imng or-dr manual)
