Imo diay gi mean kay maghimo ka ug dynamic or fillable form? I can teach you how to do that. Though you can't save a copy of the file but you can print or email.

See below image as my sample.

I suggest you better have a licensed Adobe Acrobat Pro. In my case, Im using Adobe Acrobat Professional 7 and it is bundled with Adobe Designer, a tool which enable you to create and manage dynamic forms. But for testing purposes, you can also check out the trial version offered by Adobe. Click this link --> Adobe Designer Try-out

    Steps in creating a form

    1. Go to Advanced > Forms > Create New Form.
    2. You will see a pop-up message telling you that a new form will be created in Adobe Designer.
    3. Designer will launch a "Forms Assistant" and you may choose any options either creating a form from scratch, using a template, or generating a fillable form from a static PDF or a Word file.
    4. Here's a screenshot of Adobe Designer.

Located at the right side of the Designer window is called the Library, a window where the standard tools to create a dynamic form can be located.

Once you've downloaded the trial version, feel free to explore its features. Good Luck!
