valid id with picture for example driver's license or SSS id etc
proof of income, like payslip, company certification
proof of billing like MCWD, VECO, PLDT - if you are renting, you can use the billing of the landlord
guarantor, if wife applies, husband must sign, if single - blood relative like brother parents, brother or
sister must sign
residence certificate or cedula

we also have lay away plan- meaning you get to pay the item at the regular price, not the installment price
but you only get the items when you have fully paid

as for supplementary credit card holder, it's the bank who approves or disapproves.
