Im not aware of any board having 5 PCI slots with the requirements you posted above, mostly boards today come in 2-3

Actually the trend nowadays is for 2-3PCI slots on a uATX/Micro-ATX size mobo. Even my own K9N Neo-F, which is a regular ATX mobo only has 3 PCI slots!!! So you are really gonna have a hard time looking for new mobos with more than 3 PCI slots.
