Info about Health Card

Hi. I'm a fresh graduate and I'm from Mandaue City. Another job requirement for me is to acquire my health card. I've just picked up my x-ray result today from the company. Can somebody please enlighten me the process and what are the requirements for this? Like, fees or etc? Thanks everyone!

Sauna adto ko nagkuha ug healthcard sa Mabolo Cebu City dapit queensland, ako nahibaw-an if imong employer is dili related sa foodstuffs is urine raman ang kinahanglan. dag-a nalang sad daan imong X-ray basin pangitaon.

Much better tawagan nimo daan sa pagprocess. Not sure but murag mao ni ilang number 412 8485.

Sa mga mo kuha ug Health Card sa City health please bring the following:
1. X-ray result (no need na ang film)
2. 1pc 1x1 Picture kay gamiton ibutang sa health card

Procedure:
1. Fill-up sa form (available naa sa lobby)
2. Ihatag sa Verification Area (naa ra duol sa lobby) ang form, x-ray result ug 1x1 picture then pahilbal-on ka pila imong bayran (10-50pesos food or non-food company)
3. Bayad sa 2nd floor cashier
4. Then adto sa releasing area atbang ra sa Verification area

Total processing time: 30-60mins
