doable kaayo ni nga project Boss. nakasuway nako ug PDF output. But delikado ang mga Open Online FORMS kay pwede ma.hi-jack imong DB ana specially if wala security features sa front end form and back end side.

Pwede ra man matun.an ang Form sir. review lang ka gamay sa W3Schools para sa basic then progress ka sa JQuery for front end validation.

For back-end validation, either PHP/Python/PERL imo gamiton...depende ra jud nmo. Ang PDF output ani kay pwede ra gamitan ug PDFLIB sa PHP.

Since online ni, im assuming that you are using a webhosting sa internet with cpanel(or similar) control?
