Internet + Landline only plan, no other extras

Does anyone know of any plan from any ISP that only offers Internet + Landline only? I don't need the extras that are usually bundled with the offered home plans.

Anyone who has tried applying for a business plan? Do you really need to represent a business entity to apply or can an individual be accommodated? Thanks!
