    Default QUESTION RE: PHILHEALTH & Maternity


    Somebody please tell me the process of getting MDR.

    I'll be giving birth probably late this week or next week. Our HR, which is in our Mandaluyong Head office, sent me the signed Claim form 1 and a letter (with our company's letterhead and dry seal) containing my Date of Hire, Philhealth Number, Amount of contributions for the preceding 13 months and the O.R. number each. As I have browsed through one of Philhealth info pages, I've learned that one of the requirements for Claiming discounts against hospital bills is the clear copy of MDR. I went to their service desk at Robinsons Lingkod Pinoy Stall, and asked for my MDR. The staff gave me a PMRF (Philhealth member registration form) for me to fill out and an ER2 for my employer to sign. Is this really the procedure? Because someone told me I won't have to fill out anything, and just give me Philhealth # to get my MDR.

    What are the requirements?

    1 valid ID
    your Philhealth Identification Number (PIN)
    Step by Step Guide to getting your Philhealth MDR:

    Go to any Philhealth Office near you.
    Get a MDR application form or stub from the information counter. Fill it out.
    Go to the “Fastlane” desk or window.
    Hand over your application stub and your ID.
    Wait for your MDR to be printed.
    And yes, there is no need to pay for anything. It’s FREE. It took me less than 5 minutes to get my own MDR. I guess it depends on the number of people on queue. Fortunately, I was late enough (I went there at 3:00 pm) not to get stuck in long lines.
    As in our experience, you can use your PhilHealth Card in replacement of the MDR.
