Your new company is supposed to provide you with the forms just to update your
new employer status then what they need from you is your BIR form given by your
previous employer.

As for the numbers, you need them to fill out your Employee File Form for their own
reference... Much better you ask guidance from your HR coz they're the best person
to assist you since pre-employment guidelines vary from different companies.

Have photocopies of your IDs like SSS, TIN, Pag-ibig, PhilHealth in handy.
Much better have all the IDs in one paper lang to save trees
