Who's fault is it if gagmay ang grades sa students?? Teachers or the student, itself?

It can be both.

That is why Magna Carta for student allow students to evaluate their adviser cause it is possible that an adviser have a share as to why student get low grades.

Not the case that students are the cause.

With few specifics given by TS then I can initially say that it is the student's fault. When you merely know your performance in class and you were given low grades then you have the right to ask for an explanation from your teacher. It is your fault for not asking it yet complain to anywhere else.
