Mayweather has the heart of a champion. He is determined to win by all means possible.

Floyd was winning the fight easily.
There was nothing Ortiz could do and thus I believe why he head butted Floyd out of frustration. (even with Floyd complaining, Ortiz couldn't even hit Floyd with a clean blow)
I hate Floyd, but a cheap shot is a cheap shot and I called foul on Ortiz for deliberately doing that.

Ortiz is at fault, stupid even for throwing a cheap shot and not expect someone with an ego as big as Floyd's not to retaliate. But, a cheap shot is a cheap shot and Floyd threw two. Legal, yes but cheap nonetheless.

The funny thing is, I don't hate Floyd more than I already do(I think it's already at it's limit). His actions, I don't conform to but I expected from him. The excessive smiling from Ortiz is what really bothered me. He smiled so much you thought he won the fight. That he didn't lose his title. That he didn't lose the biggest fight of his life. Smiling too much that I still have doubts if this was a WWE match.

To the question of who is really to blame? I'd say all three men in the ring.
