Could be any one of these reasons:
The one needing healing did not have enough faith to be healed OR the ones trying to do the healing did not have enough faith for it to happen.
God has a Purpose for this amputee to fulfill in this condition for His sake, which is always for the sake of others in need of understanding greater concepts of life.
Jesus once said that if your right arm offend you, cut it off. Better to go into heaven maimed than into hell with all your limbs. I'm not at all suggesting that the person was offended by the limb that was amputated, but Jesus did not think a complete body important to the learning of greater concepts of life and soul. (Matthew 5:29-30)
