Guinea Pig 2 is by far the most repulsive film I've seen, but there's virtually no storyline and I wouldn't recommend watching it as it's just plain disturbing. I Saw The Devil on the other hand is an excellent film in many respects and the gore and violence while shocking never crosses the line into voyeurism.
