Received my Medical Notice yesterday... and received the medical forms today. It says that I have 30 days to complete or else the application will be refused. Hoping for a smooth process from here...

Same here, I just received my PER via snail mail 2 days ago. It was also dated Dec. 8, 2014. I submitted my application last August 13, 2014.
