nope!! the requirements should be notarized @ prc are examination form(pink form) and the notice of admission( these requirements however were only given upon submission of your TOR and Summary of cases) , since they will be using a specialized machine/laser to put a code on it....PLEASE BRING THOSE ORIGINAL CASE SHEET ka2 naa sa OR/DR manual kay mo question raba na cla sa validity sa imo cases esp. if doubtful cla sa signature....

your summary of cases can be notarized either @ prc or outside(ako outside)... but I suggest to have it notarized outside SAMOK UG GUBOT PA SA LOKOT ang PRC, but then again if sigurista kaau ka then go for it TASTE THE ADDITIONAL LIMBO!!! hahaha

hehe... kmi sauna hadlok sd nga tanawon ang mga case manuals ky wla na ang uban pero wla mn gicheck labi na kng dghn tao samot na kng hapit na last day sa pg file. kng wla na inyong mga original cases forms ayaw mo apply syo ky bsi checkon/verify.

1 last thing: para dghn mog stock knowledge... Dont ever memorize... just understand...!!!

I suggest start reading your books. Med Surg, Psychia, and Public Health Nursing. Kana sila 3 mka basa ka ana dako na ka ug chance gyud, then understand your Funda and Jurisprudence Pasar na kaau ka
